Registering with the surgery
To register with the surgery:
- fill out an online registration form and an online New Patient Health Questionnaire
- download and complete a New Patient Registration Form
- visit reception when we are open and collect a paper registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Please note:
Ashwell and Bassingbourn Surgeries fall under the umbrella of the Hertfordshire and West Essex Integrated Care Board (ICB) and therefore your care is funded by this ICB. Even if you reside within Cambridgeshire, it may not always be possible for us to refer you to Addenbrookes because our ICB may not have a contract to support referrals to Addenbrookes hospital.
Currently we cannot refer to Addenbrookes for the following clinics / specialities: pain clinic, neurosurgery (back issues), Adult and Paediatric mental health services.
Named GP
All patients are registered with Ashwell Surgery, under what is known as a pooled list.
You can talk to or make appointments to see any of our doctors or nurses.
If you have a preference and would like to request a particular doctor at the practice to be your named GP please talk to one of our receptionists or indicate it on your appointment request form
