Registering with the surgery
To register with the surgery:
- fill out an online registration form and an online New Patient Health Questionnaire
- download and complete a New Patient Registration Form
- visit reception when we are open and collect a paper registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Practice Sites
The practice operates from two locations: Ashwell Surgery (our main branch) and Bassingbourn Surgery (our branch site).
Appointments are allocated based on clinical need and availability across both sites. Patients registering with the practice should be aware that they may be required to attend either location for appointments and services. Acceptance of registration with the practice includes agreement to travel to either site when appointments are offered.
Patients may request appointments at a preferred site; however, this may limit appointment availability and could result in a longer waiting time.
Named GP
All patients are registered with Ashwell Surgery, under what is known as a pooled list.
You can talk to or make appointments to see any of our doctors or nurses.
If you have a preference and would like to request a particular doctor at the practice to be your named GP please talk to one of our receptionists or indicate it on your appointment request form
